Right now, we have an exciting opportunity for a Clinical Pharmacy Specialist to deliver clinical pharmacy services to pediatric and neonatal populations at the Santa Monica UCLA Medical Center. As a key member of a multidisciplinary team, you will be responsible for reviewing patient drug therapies per direct consult or patient rounds to identify drug-related issues and makes recommendations to optimize patient care. You will:
Adjust patients' drug therapy regimen utilizing appropriate pharmacokinetic and pharmacodynamic principles.
Detect and document adverse drug reactions.
Interview and counsel patients and their families.
Provide medication reconciliation and discharge counseling to patients and their families.
Coordinate patient care activities with the Pharmacy and Pediatric/Neonatology services.
Serve as a drug information resource for physicians, nurses and other health care providers.
Facilitate investigational drug studies.
Deliver educational programs on medication use for health care professionals.
Precept post-graduate pharmacy practice residents and students.

We're seeking a dedicated clinical leader with:
California Pharmacist Licensure.
Doctorate of Pharmacy Degree from an ACPE-Accredited College of Pharmacy.
Completion of PGYI Pharmacy Residency.
Completion of PGY2 Pharmacy Residency in Pediatrics or equivalent experience.
Board certification in specialty preferred.
